The Sea Cloud arranges for a doctor to be on board for any of its cruises. I imagine some arrangement is made, either paying the doctor a given fee, or perhaps offering a free cruise as compensation. I don't know the actual details, but I'm sure the passengers take comfort in having a doctor on board.
A small infirmary occupies a cabin on board and the doctor has general visiting hours early each morning. You can tell by the darkness in this photo that it must be quite early. As I passed by and took my flash picture, the doctor was startled to see someone up and about.
The good doctor's wife accompanied him on the cruise, so that was a good arrangement for them. The doctor and his wife went ashore on many of our outings, so they traveled much like a paying passenger.
