First class compartments do not have a private toilet or shower. Passengers share facilities in the shower car. The primary interest here is a lower cost for the trip, but the potential negative is dealing with the sharing concept.
The toilet or shower may not be immediately available to the passenger, and the cleanliness is largely dependent on the people using the facilities before you. Of course, the car attendant would check on the cleanliness of the shared facility periodically.
Functionally, the share toilet had good facilities - it just wasn't in your individual compartment.
